At Atwell Primary School, your child’s health, safety and wellbeing are our priority. To help us support your child’s medical needs, we ask families to provide up to date health information when enrolling and throughout the school year.
What Parents Need to Provide
When your child enrols, you will be asked to share:
Australian Immunisation Register (AIR) statement
A completed Health Care Summary outlining your child’s medical needs
Management and Emergency Response Plans, if your child requires specific support at school
If your child has complex medical needs, you are welcome to request a meeting with the school to discuss their care.

Management and Emergency Response Plans
These plans help staff understand your child’s day to day health needs and what to do in an emergency. Plans may include:
Daily care or support routines
Emergency procedures
Staff training requirements
Medication information (dosage, storage and timing)
Signed medical authorisation, when required
Plans should be kept up to date each year, or sooner if your child’s needs change.
Plan Templates
The school provides templates for common medical conditions, including:
Anaphylaxis and allergies
Seizures
Asthma
Diabetes (using Diabetes WA templates)
Activities of daily living
General health care needs

Medication and Equipment at School
If your child needs medication during the school day, please ensure:
Medication is in its original packaging, clearly labelled with your child’s name and within expiry
You have completed the school’s Medication Authorisation Form
Families are responsible for providing any required medical equipment in good working order.
